    • Documented proof of two Varicella vaccines, serological evidence of immunity, or verification of a history of varicella or herpes zoster (shingles) by a healthcare provider.
    • Valid 10 years from the administered date. No declinations or attestations accepted.
      • Additional Details
      • Must be AHA.
      • Completed within a year of start updated yearly. Minimum 80% score. Only 2 attempts allowed.
