Registered Nurse I Job Specialty: Home Health Job Duration: 13
Weeks Shift: Day 5x8-Hour (08:00 - 17:00) Guaranteed Hours: Not
guaranteed Experience: Minimum 1 full year of Adult Home Health or
Hospice experience License: Must have a valid nursing license in
hand Certifications: CPR & BLS required Must-Have: - Proficiency in
HomeCare HomeBase (HCHB) and OASIS - Reliable transportation, valid
driver's license, and agency-required liability insurance -
Comfortable traveling up to 75 miles from the home clinic Job
Description: - Provide, coordinate, and direct home nursing care as
per physicians--- orders and agency policies. - Supervise and
evaluate Home Health Aides and Licensed Practical/Vocational
Nurses. - Attend home visits and periodic office meetings. -
Responsible for documentation and travel activity points, with
productivity expectation of 30-32 points per week. - Handle various
types of cases such as Start of Care, Wound Care, Follow-up Visits,
etc. - Participate in on-call rotation every 5 weeks from Friday 5
pm to the following Friday 8 am. - Must complete documentation
within 24 hours of a patient appointment. - Be willing to float to
a nearby clinic if required to meet financial needs due to a low
patient census. On-Call Requirement: - Participate in on-call
rotations, as discussed during the interview. Additional Notes: -
Must not be a current direct employee of the agency applying as a
traveler. - Open to working with local and traveling professionals
at the same rate. - Account managers will review financial concerns
if weekly productivity points are not met. Candidates must confirm:
- Awareness that this is a home health assignment requiring daily
travel to patients' homes and weekly clinic meetings. - Possession
of a vehicle, driver---s license, and auto insurance. - Comfort
with drives longer than 20 minutes between patient visits and
