Job Description:
A company is looking for a Case Manager Workflow Lead.
Key Responsibilities:
Serve as a lead and resource for Discharge Planners (DCP) and the
multidisciplinary team
Participate in assessing patients' clinical and psychosocial needs
and facilitate referrals to appropriate services
Communicate with patients, families, and healthcare team members
regarding care plans and treatment progress
Required Qualifications, Training, and Education:
Bachelor's Degree in Nursing or Master's Degree in Social Work
Four years of acute care case management experience in an acute
healthcare environment
Must be an Accredited Case Manager or Certified Case Manager upon
start
Basic Life Support certification through the American Heart
Association upon start
Current RN license or Social Work license eligible to practice in
the Commonwealth of Virginia
